| To begin, please provide the following documentation: |
| 1. Download our Carrier Package. Fax signed and completed forms to (251) 743-5278. |
| 2. Fax a copy of your cargo and liability insurance. Have your insurance company send us a hard copy naming Hornady Logistics as an additional insured under these policies. |
| 3. Fax a copy of your authority. |
| 4. Fax a current W-9 form. |
